چکیده

Breast cancer is the most common malignant disease in the female population. Significant psychological, social, and sexual morbidity can follow mastectomy for treatment of breast cancer owing to the realization of the life-threatening disease as well as impaired body image [1]. Breast reconstruction improves the patient’s self-confidence after the loss of a breast from cancer or trauma [2]. Of the breast reconstructions using autologous tissue, the most commonly used flaps involve a deep inferior epigastric perforator (DIEP) flap or free muscle-sparing transverse rectus abdominis myocutaneous (MS-TRAM) flap, both of which allow a sufficient volume of tissue. However, owing to the long transverse scar between the anterior superior iliac spines (ASIS), patients hesitate to choose breast reconstruction using a free MS-TRAM or DIEP flap. Furthermore, for a traditional reconstruction using a free MS-TRAM flap, the donor site must be dissected to the subcostal area in order to close the wound after elevating the flap and form the neo-umbilicus. As a result, there is an increased risk of donor site morbidity, such as pain, hematoma, or seroma. Moreover, a significant amount of the flap is discarded. In an attempt to address these shortcomings, we developed a novel method using a mini-abdominoplasty for flap elevation, and Tae Hoon Kim, Kyung Won Minn, Ung Sik Jin

BREAST RECONSTRUCTION USING TRAM FLAP: PROSPECTIVE OUTCOME AND COMPLICATIONS

ABSTRACT Background: The transverse rectus abdominis musculocutaneous (TRAM) flap remains the gold standard for postmastectomy reconstruction either immediate or delayed. However, transfer of TRAM flap can be associated with donor site morbidities and complications in flap. A successful reconstruction consists of careful patient selection, surgical technique and meticulous preoperative plannin...

Scars and perforator-based flaps in the abdominal region: a contraindication?

BACKGROUND Although multiple strategies for autologous breast reconstruction exist, a vertical midline scar in the abdominal wall as a result of previous laparatomy or abdominoplasty represents a major surgical challenge. To date, little research has been conducted on the regeneration potential of the abdominal wall's superficial vascular, perforator and choke vessel system after surgery using ...

Satisfaction following Unilateral Breast Reconstruction: A Comparison of Pedicled TRAM and Free Abdominal Flaps

BACKGROUND The purpose of this study was to compare patient satisfaction following unilateral pedicled transverse rectus abdominis myocutaneous (TRAM) and free abdominal flap reconstruction. METHODS Patients who underwent unilateral breast reconstruction using pedicled TRAM or free abdominal flaps (muscle-sparing TRAM or deep inferior epigastric perforator flap) and completed the BREAST-Q wer...

Unilateral and Bilateral Breast Reconstruction with Pedicled TRAM Flaps: An Outcomes Analysis of 188 Consecutive Patients

BACKGROUND The abdomen remains a popular donor site for autologous tissue breast reconstruction. Recently, however, some authors have questioned whether the pedicled transverse rectus abdominis myocutaneous (TRAM) flap should remain a first-line reconstruction option. METHODS Between 1998 and 2009, 188 women underwent breast reconstruction with pedicled TRAM flaps by the senior author (J.A.A....

Nipple Reconstruction Using the C-V Flap Technique: Long-Term Outcomes and Patient Satisfaction

BACKGROUND Nipple creation using the C-V flap technique is often the final step in breast reconstruction. The aim of this study was to subjectively and objectively assess the cosmetic outcomes and satisfaction of patients undergoing C-V flap nipple reconstruction. METHODS Subjective assessments of patient satisfaction with the neo-nipple were recorded by visual analogue scoring (VAS; 0...
